There are tremendous benefits of documenting a good patient history. Over 80% of the time, a detailed patient history will yield an accurate diagnosis. Our patient forms are available below. They can be completed on line and returned before your first visit (preferred) or you may print them and bring on your first visit. The more information Dr. Carrick has before a first appointment, the more efficiently she can work with you to return you to your optimal health.
